New patients should complete a health history questionnaire online through our patient portal prior to your visit. Upon your arrival at the office, the staff will verify and update your information for accurate insurance billing. Please plan to arrive 10 minutes prior to your scheduled appointment time.
Our front desk staff will ask to see your photo ID and insurance card at each appointment. They will review the insurance information you provide to ensure the data we have on file is current and accurate. MyMenopauseRx will submit your bill to your insurance company as a courtesy. Please be aware, if you arrive to your appointment without your photo ID or insurance card, your appointment will be rescheduled.
Please call 1-630-480-0428 during regular business hours. If you have a life-threatening emergency, please call 911.
If you need to be seen for an emergency, please go to your closest emergency room. The emergency room physician will contact the physician on call. Our physicians are on call for emergencies only and answer after hour calls only from the emergency room. All other questions or health complaints can be handled during normal office hours. If you have an urgent matter, we will always try our best to squeeze you in if the clinical staff determines you need to be seen.
If you need a refill of a prescription, we recommend you request a refill via the patient portal or have your pharmacy send us a request. If our clinical staff requests you make an appointment to obtain your refill, please do not argue with the clinical staff. They are just following the doctor's recommendations.
All normal test results will be communicated via the patient portal. If a test is abnormal, the physician will request you make an appointment. The physician will discuss your results and any treatment recommendations at your appointment.
If you prefer to speak to your physician by phone, we recommend you make an appointment for a virtual visit. For a more immediate response, we recommend you send us a message via our patient portal.
Your appointment time has been reserved in the physician's schedule specifically for you. Please be advised that we require 24 hours notification for appointment cancellations. If you must cancel your appointment less than 24 hours in advance, you will incur a $100.00 cancellation fee. We cannot offer your time slot to anyone else if we do not have 24 hours notice.
If you arrive late for your appointment, we will do our best to accommodate you. Please understand that this is not always possible. If you know you are running late, please call the office to alert the staff.
Your insurance coverage is a contract between you and your insurance company. Please check with your health insurance plan by calling the number on the back of your insurance card to verify physician participation. Insurance plans are always merging and changing. Please note that insurance company websites may not be up-to-date.
As a courtesy to our patients, MyMenopauseRx will bill your insurance carrier. Co-payments are collected at the time of service. If your deductible has not been met, all services that are not considered prevention are usually applied to your deductible and you will receive a bill from our office.
