She met her west coast husband in medical school and they both decided to call Chicagoland their home. After completing her residency training in Obstetrics and Gynecology at the University of Illinois in Chicago, Dr Hanna discovered her true passion was practicing Gynecology and Women's Healthcare, not delivering babies. She went on to spend most of her 25+ year career in private practice as a minimally invasive gynecologic surgeon. As her patients aged, she was faced with menopause questions she was unable to answer confidently. She was one of the 80% of the physicians who obtained little menopause training in her OB/GYN residency. Dr Hanna enhanced her knowledge by becoming a North American Society Menopause Practitioner to better serve her patients. She retired her scalpel to focus on transforming women's lives at perimenopause and menopause. MyMenopauseRx was launched in April, 2021. Dr Hanna and her physician husband, are grateful to have raised 3 incredible children and a 90lb German Shepherd. They love to spend their free time with their family - cooking, exercising, and traveling. # Steven Hanna ### MMRX Founder & Chief Technology Officer Steven Hanna grew up in a small, midwestern town before moving to Silicon Valley to be a software engineer at a large enterprise data company. While classicaly trained in the art of software, Steven quickly became a critical member of the enterprise sales cycle, often being the product's subject matter expert and engineer tasked with building customers requests in order to close large deals. Steven's software is deployed across tens of thousands of customer machines, helping process petabytes of information every day. His ability to quickly turn business requirements into seamless product experiences allow his teams to quickly pivot and iterate on their ideas. He pursued his undergraduate studies, medical education, and Obstetrics and Gynecology residency at the University of Illinois. However, it was a move to New Orleans, where his wife's family hailed from, that truly shaped his career. Over the next 27 years, Dr. Gelman established a thriving Ob/Gyn practice. As his patient population matured, he found himself transitioning from delivering babies to providing comprehensive care for women navigating menopausal changes. Seeking to stay at the forefront of medical knowledge, he earned a Master's degree in Public Health from Yale University. Dr. Gelman's passion for menopause care led him to collaborate with his friend and colleague, Dr. Hanna, at MyMenopauseRX. She completed the Fellowship in Integrative Medicine at the University of Arizona with Dr. Andrew Weil and trained in medical acupuncture and Mindfulness-Based Stress Reduction. Throughout her career, she has cared for women from adolescence through old age and enjoys the deep bonds forged with her patients. During your telemedicine consultation with the Clinician, details of your medical history and personal health history may be discussed through the use of interactive video and /or other telecommunications. You will be asked to verify your identity by uploading a copy of your driver's license or state-issued identification into our platform. Our services will only be offered in states where Clinicians are licensed to practice. You understand that the laws that protect the privacy and security of health information (HIPAA) apply to telemedicine and you have read, understood and agree with MyMenopauseRx privacy policy which describes these protections in more detail. By accepting this agreement, you acknowledge and agree that the Medical Group and/or Clinicians may send you messages, reports and emails via the service regarding your diagnosis and/or treatment. Your further understand and agree that it is your sole responsibility to monitor and respond to these messages, reports and emails and that neither MyMenopauseRx, the Medical Group nor any Clinician will be responsible in any way and you will not hold MyMenopauseRx, the Medical Group or any Clinician liable for any loss, injury, or claims of any kind resulting from your failure to read or respond to these messages or for your failure to comply with any treatment recommendation or instructions from the Medical Group or your Clinician. ### Electronic Medical Record MyMenopauseRx Electronic Medical Record (“EMR”) is created for you to enter, store and access your personal health information online, including medical history, current menopause related health conditions, symptoms, complaints, allergies and medications, and for the Clinician to record the results of his or her medical encounters with you in accordance with his or her obligations under applicable state and federal law with regard to creating and maintaining your record. Any information you provide during your video or message consultation becomes part of your MyMenopauseRx EMR as entered by your Clinician. We do not video record your private consultations. As part of your care through our telehealth services, we may utilize technological solutions including a listening device to record your conversation with your care team. The listening device uses artificial intelligence (AI) tools to create notes about your visit to enhance clinical quality, streamline documentation, and improve your overall healthcare experience. The data processed by AI systems are encrypted and stored securely in accordance with HIPAA and applicable federal/state privacy laws. The recordings are not part of your medical record. MyMenopauseRx can not give you the recordings as part of a medical records request. You may request that your session not be recorded or AI not be used. We strive to ensure the accuracy of AI-assisted tools and your clinician will review, edit and finalize visit notes before they are added to your medical record. You agree to provide accurate, truthful, current and complete information about yourself, family history, medical history, symptoms, complaints, allergies, medications and preferred pharmacy. You agree to review and update such information prior to each video or message visit and as needed to keep it accurate, current and complete. By requesting a consultation through MyMenopauseRx's medical services, you agree to disclose the contents of your complete MyMenopauseRx EMR to the Clinician who will conduct your consultation. We recommend that the user read and share their consultation documents with their personal health clinician. ### Limitations of Use You will not use the website or Services in any unlawful way and/or for any unlawful purpose in any way. By using our Services, you may choose to be seen as a self-pay patient or choose to use your health insurance if your plan is accepted. If you choose to use your health insurance, your final out-of-pocket cost will be determined solely by your insurance plan’s coverage, co-payments, co-insurance, and deductible status. MyMenopauseRx will submit a claim to your insurance plan on your behalf, and any balance not covered by your plan will be your responsibility. We encourage you to contact your insurance provider to verify your benefits and understand your potential financial responsibility. If you elect to be seen as a self-pay patient, you acknowledge that you will not have the direct ability to utilize your medical insurance for payment at the time of service. MyMenopauseRx does not accept assignment of benefits for self-pay patients. You will be solely responsible for submitting any documentation requested by your health insurance plan, health savings account (HSA), or flexible spending account (FSA) for reimbursement. MyMenopauseRx makes no guarantee of reimbursement by any payer. **Patient Responsibility**. You acknowledge and agree that you are responsible for all changes for services provided to you which are not covered by your health plan or which you are responsible for payment under your plan, regardless of treatment outcome. To the extent no coverage exists under your plan, you acknowledge that you are responsible for all charges for services provided and agree to pay all charges not covered by your plan. For payment, we accept credit or debit cards issued by U.S. banks, and all prices are set in United States Dollars (USD). If a credit card is used for a transaction, MyMenopauseRx may obtain a pre-authorization for an amount up to the full amount of the payment. If you wish to use a different credit card or there is a change in your payment method, you must update your information online at the time of placing a new order through Square, our third-party credit card processor. A new Payment Method may temporarily delay your ability to make online payments or use Services while we verify your new payment information. By providing a credit card or other accepted form of payment by MyMenopauseRx, you expressly agree that MyMenopauseRx is authorized to charge your payment method for any applicable fees associated with services you receive, including consultation fees, lab fees, and any applicable taxes. You agree that this authorization to charge to the payment method remains in effect until you cancel it in writing, and that you will promptly notify MyMenopauseRx of any changes to your payment information. You certify that you are an authorized user of the Payment Method and will not dispute charges for the "Services" that correspond to consultation, lab fees or other provided services. You acknowledge and agree that fees for consultations or lab services may increase at any time. Furthermore, you represent and warrant that if you are making online payments that (i) any credit card, debit card and bank account information you supply is true, correct, complete and accurate, (ii) charges incurred by you will be honored by your credit/debit card company or bank, (iii) you will pay the charges incurred by you in the amounts posted, including any applicable taxes, and (iv) you are the person in whose name the card was issued and you are authorized to make purchases or other transactions with the relevant credit/debit card and credit/debit card information. MyMenopauseRx is not a bank. We contract with a third party (Square) for credit card processing on our behalf. You understand that you are fully responsible for payment. You understand if you have an unpaid balance to MyMenopauseRx and do not make satisfactory payment arrangements, my account may be placed with an external collection agency. You will be responsible for reimbursement of the fee of any collection agency, which may be based on a percentage at a maximum of 35% of the debt, and all costs and expenses, including reasonable collection and attorney's fees incurred during collection efforts. In order for MyMenopauseRx or their designated external collection agency to service my account, and where not prohibited by applicable law, you agree that MyMenopauseRx and the designated external collection agency are authorized to (i) contact you by telephone at the telephone number(s) you are providing, including wireless telephone numbers, which could result in charges to you, (ii) contact you by sending text messages (message and data rates may apply) or emails, using any email address you provide and (iii) methods of contact may include using pre-recorded/artificial voice message and/or use of an automatic dialing device, as applicable. If a Clinician prescribes a medication, she/he will do so when medically indicated in their sole professional judgment. There is no guarantee a clinician will issue a prescription. You understand the lack of access to all of your medical records or incomplete, untrue, inaccurate, incorrect medical history may result in adverse drug reactions or allergic reactions or other judgment errors that may or may not be foreseeable. You agree that any prescription that you acquire from the clinician shall be solely for your personal use as directed by your Clinician. You agree to fully and carefully read all the provided product information and labels provided with your prescription from the pharmacy and contact a physician or pharmacist if you have questions. If you choose to cancel your appointment 72 hours prior to your appointment time, a refund will be made to the credit card you utilized to schedule your appointment. There are no refunds for missed or cancelled appointments unless done 72 hours prior to your appointment time. Our Medical Group and clinicians practice evidence-based medicine. There are no refunds for self-pay visits completed that did not meet your expectations. **Insurance.** Our Medical Group and clinicians practice evidence-based medicine. A printed version of this agreement and of any notice given in electronic form shall be admissible in judicial or administrative proceedings based upon or relating to this agreement to the same extent and subject to the same conditions as other business documents and records originally generated and maintained in printed form.
